Seri Wajah Muslimah Aesthetic has developed this Privacy Policy to outline how we collect, process and disclose information that we obtain when you access or use our services, including our websites and applications, including information about you.     • Image and photograph: Recording of your image and photograph via CCTV cameras installed at our premises, during any of our events, third party events and for medical records purposes; and

We may collect personal data either from you, your authorized representatives, third parties, or any publicly available sources which may include but not limited to the following:-

    • Registration form, consent forms, purchase forms or any other forms that you or your representative fill in at our premises;
    • When you contact us in person, by a phone call, email or letter and over the counter;
    • Any surveys, questionnaires, competitions, contests, offers, or promotions done by SWMA or via an appointed third party
    • Any third parties connected with you such as your employer/ potential employer, agents, insurance companies, other healthcare facilities/ providers;
    • Your visit at any of our premises;
    • Your attendance at any of our events or events organised by third party;
    • From our websites;
    • Job application and interview;
    • From your interaction with us via social media or interactive applications including but not limited to Facebook, Twitter, Instagram,  and etc; 
    • From such other sources where you have given your consent for the disclosure of personal data relating to you, and/or where otherwise lawfully permitted.

    • To ensure continuity of treatments at our premises;
